Outline of the Web Form available to Partners

Please enter the required data into the following form. The data is first submitted and presented to you again to be checked for bugs. After checking the data has to be submitted again to be stored on the system.

General data

Time period of submitted data:

Incident and Report related data

1) Total number of requests:			All non-automated (i. e. IDS) requests received by the team, including all non-incident handling requests.
2) Incidents opened:			All incidents opened that month.
3) Incidents closed:			All incidents closed that month.
4) Inbound incident related communication:			All reports (Email, Fax, Phone, ...) received by the team which constitute part of an incident, counted at 2.
5) Outbound incident related communication:			All reports (Email, Fax, Phone, ...) sent by the team which constitute part of an incident, counted at 2.

Organizations involved

6) Organizations involved:			All organisations, which were involved in the incidents counted at 3, each site should only be counted once per incident, per month).

Amounts of time spent

7) Time spent:			Total time in hours spent on incident response.

After the submission of the form data the values are displayed again to be checked. Only after the final submission the data will be stored on the system.
